Experiencing Homelessness in Suffolk County or Nassau County?

Coordinated Entry (CE) has a primary goal of providing easy access to housing and services for people experiencing homelessness in a shelter or on the street, including housing needs assessments and referral to CoC permanent housing programs when eligible and prioritized. There is also a designated DV CE with supports tailored for people actively fleeing domestic violence, including safety planning.

CE can also help provide information and make referrals to local resources to households at risk of facing homelessness or otherwise unstably housed that can help them maintain or obtain housing.

Fleeing violence in a DV shelter, shelter/motel through DSS, on the street, or in the community (couch surfing, staying with friends or family, living with abuser or other unsafe housing)

Fleeing DV while residing in the community and/or at-risk of being homeless in shelter or on the street: Limited- Safety planning, legal advocacy, prevention/housing problem solving, help accessing shelter if needed

 

At-risk of Homelessness- Facing Imminent risk of becoming homeless within 30 days (e.g. issued an eviction letter)

 

Access CE

 

Limited - Safety planning (DV CE), legal advocacy housing problem solving, help accessing shelter if needed

CoC Permanent Housing Resident- Emergency Transfer Plan (ETP)- Was stably housed in CoC permanent housing but now at-risk of violence and becoming homeless again due to actively fleeing DV

 

 

Time-Limited- Safety planning (DV CE), housing placement and coordination and other VAWA eligible activities to keep household safe until transferred to other housing.

Otherwise unstably housed- not homeless either living on the street or in DSS emergency housing, and not actively fleeing or attempting to flee.

 

 

 

Not eligible for CE or DV CE; refer to CoC website and/or 2-1-1 to navigate resources on their own

None/One-Time- redirected to other community resources

Households outside of Nassau or Suffolk counties- Households experiencing homelessness or domestic violence in areas other than Nassau or Suffolk counties.

 

 

 

Not eligible for NY-603 CE or DV CE; refer to resources in their area to navigate resources on their own, including national DV hotline

None/One-Time- redirected to community resources in their area
