• A Continuum of Care (CoC) is a network of community partners working together to plan and coordinate the local homeless response system in a given region, our region being NY-603 which covers all of Nassau and Suffolk Counties.

 

• The CoC brings together organizations, systems, resources, data, and people with lived experience to identify community needs, address gaps, strengthen services, and improve connections to housing and support.

 

• Ultimately, the CoC works to create a more coordinated and effective system that addresses, prevents and ultimately works to end homelessness

• Since 1995, LICH has been the CoC lead entity for our regional CoC, which covers Nassau and Suffolk counties. Our CoC is one of the largest across the U.S. in terms of homeless and funding rates, comparable to many U.S. cities
