The Long Island Continuum of Care is designated by the Department of Housing and Urban Development to oversee the regional strategy for ending homelessness. The Continuum of Care envisions a community in which all members have access to safe, decent, and appropriate housing options.

Outreach Resources

Housing for New Hope's Assertive Engagement (AE) Team provides intensive case management to the uninsured homeless in Durham who have difficulty remaining in mainstream treatment. Relationships built by the Assertive Engagement (AE) Team and capable volunteers lead to housing, improved health, and connections with ongoing services. Strong partnerships in the community are able to connect clients with mental health services when needed.
