Housing Needs Assessment

The local assessment tool focuses on and barriers to exiting homelessness as a guiding tool to determine who will be prioritized for permanent housing resources first. 

  • Meeting the person where they are, in a safe/preferred location of their choosing (person-centered)

  • Completing an intake form with information about housing and service goals (housing-focused)

  • Completing a housing and services preference form (person-centered)

  • Completing a formal housing plan (critical time intervention)

  • Completing a trauma recovery exercise (trauma-informed care)

  • Completing any necessary consent forms for required medical documentation related to obtaining housing (service needs)
