What is Street Outreach?

Street outreach teams go out into the community to connect with people living unsheltered and try to connect them to services. Some teams can provide essential items, such as food, clothes, and/or toiletries. Others provide housing-focused case management, substance use or mental health support.

Chronic Homelessness means that someone has been living on the street or in shelter for longer than one year and has a permanent disability
